We investigated the effects of chronic renal failure on the pharmacokinetics of theophylline. Four chronic renal failure patients (mean age, 52 yr) and four healthy control subjects (mean age, 52 yr) were given a single intravenous dose of theophylline, 14 mg/kg. Serum theophylline concentrations were determined by high-performance liquid chromatography. Serum creatinine concentrations were also measured. C(max) (mean +/- SD) for the control group was 152 +/- 59 microg/mL and for the renal failure group was 101 +/- 36 microg/mL, a statistically significant difference (P = 0.008). The mean clearance was 51 +/- 13 mL/min for the control group, but 38 +/- 8 mL/min for the renal failure group, which was not statistically significantly different (P = 0.07). Serum creatinine concentrations correlated with C(max) and mean serum theophylline concentrations in both groups. There was a high degree of intersubject variability in both C(max) and mean serum concentrations in the control and renal failure groups. Our data indicate that a reduction in renal function can significantly affect the pharmacokinetics of theophylline. of the situation before us, we believe that the following language from Thompson v.
